What to Expect
When you contact Laurel Street Chiropractic & Rehab to book your first appointment, we’ll send you a link so that you can complete your new patient paperwork at home. Simply print out the forms and bring them with you. If you’d rather take care of the paperwork here, that’s fine too!
We suggest wearing relaxed, comfortable clothing and bringing your identification, insurance card, and any previous X-rays or MRIs you may have done.
NEW PATIENT FORMS
Your First Visit
Our friendly staff will greet you when you enter and answer any of your questions. We’ll get any paperwork and insurance taken care of, then we’ll take your vitals before you meet with your chiropractor. During your consultation, your doctor will talk with you about your health history, what’s currently going on, and what your goals are for care. Then, with your consent, they’ll move forward with in-depth examinations, including testing your reflexes and range of motion, as well as other orthopedic, neurological and postural assessments.
If they deem X-rays needed, they will refer you out for those. There is an imaging clinic a short distance from our office. Your doctor will explain their findings briefly and, if appropriate, will begin care at this visit (with your consent.) You should plan to be with us for about 45 minutes to an hour.
Payment Options
We are in-network with BlueCross/Blue Shield, Cigna, Aetna and TriWest (unfortunately TriCare currently does not cover chiropractic care). Our staff will verify your coverage for you and explain your benefits and our pricing, so you know exactly what to expect. You may use your HSA or FSA, and we also accept ChiroHealthUSA®. For workers’ compensation and personal injury cases, we directly bill insurance. Ask about our discounts for military personnel and first responders.
